  • I am “in-network” with:

    Aetna

    First Choice Health

    Kaiser Permanente NW

    Lyra

    PacificSource

I am “out of network” with all other insurance companies.

Most insurance plans offer out-of-network benefits for therapy. For clients wishing to use their out-of-network insurance benefits, I can provide you with the necessary document called a “superbill” for you to submit to your insurance company for reimbursement. The full cost of the session is required at the time of service. Please call your insurance company to ask about your out-of-network benefits.

Please check your coverage carefully; consider asking the following questions:

Do I have mental health insurance benefits?

What type of services are covered (individual, family, couples, group)?

Do I have a deductible due before coverage begins? If so, how much is it and how much has been applied to it so far this year?

How many sessions per year does my health insurance cover?

What is the coverage amount per out of network therapy session?

Do I need a referral from my primary care physician to be eligible for mental health coverage?

Is there a restriction on the license category of my mental health provider?

    STANDARD NOTICE

“Right to Receive a Good Faith Estimate of Expected Charges” Under the No Surprises Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your health care provider, and any other provider you choose for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 360-836-4334

  • The first session is spent getting to know you and finding out more about what you are struggling with. The focus will be on learning about what is causing you distress in your life and how you would like things to be.

  • The first step is to contact me or reach out to 36-836-4334 or email me at arabeth@threecreekscounseling.com for a free phone or via consultation. If I am unavailable, I will call you back within 1-2 business days.

The point of the free phone consultation is to briefly understand your goals for treatment and determine if I’m the right fit for you.

At this time you can ask me any questions you may have as well as explore your goals for treatment.

If we’re a a good fit, we will coordinate availability and get you scheduled. If I’m not the right fit for you, I will provide you with referrals to other providers or services that may be more effective for your situation.
